Transition from rehabilitation settings to community via the NDIS for people with complex needs

Session Overview:
Rehabilitation and recovery are vexed issues for the NDIS. There are areas of overlap in the responsibilities across health systems and the NDIS, and challenges for rehabilitation teams in ensuring continuity in recovery for patients as they move into the NDIS, particularly those with complex support needs, including those with communication and behaviour support needs. Deb Farrell and Alan Blackwood will explore the key issues and provide practice examples of planning for transition of people with very complex support needs from rehabilitation settings to community via the NDIS.

What will be covered:
  • Guide to preparing for NDIS planning and community transition with the person with complex needs
  • Government targets for addressing the issue of young people in residential aged care (YPIRAC) and what this means in practice
  • Case studies of people with very complex support needs transitioning back to the community after rehab to illustrate some of the challenges and outcomes
  • Report on NDIS funded rehab/disability services interface project the Alliance conducted in NSW in 2019

Meet our Presenters:


Deb Farrell

With a background in health, Deb has worked in the community and disability services sectors for many years.  Deb worked with community organisations examining strategic directions and organisational capabilities needed to work within the new NDIS environment and this included her work in the Barwon, ACT and Hunter NDIS trial sites.
 
Deb is currently the Senior Adviser to the YPINH Alliance’s Cross Sector reform work. Funded by the Victorian Government, this work supports young people requiring service responses from multiple programs in their transition to the NDIS; and the cross-sector collaboration needed to deliver better integrated services for this group. 


Alan Blackwood

Alan has worked in disability advocacy and policy development for over 30 years in individual support, policy and advisory roles, and has been closely involved with the development of the NDIS through a number of state and national advisory bodies.

Alan managed the national policy team at MS Australia prior to working at the YPINH Alliance and is the current chair of Children with Disability Australia. Alan also actively campaigns for the full rollout of the National Injury Insurance Scheme.
