Session Overview:
Patient Stories involves interviewing patients directly to gather their unique insights to gain a better understanding of the amputee’s experiences and perspective of our service. Allowing patients to share with us the story of their journey across the continuum of care can help identify gaps within our service, inspire patient centred care, celebrate good practice, and foster learning to allow us to enhance our patient experience by delivering an improved service.

Meet our Presenter:



Siobhan Jackson

Siobhan is an Amputee Nurse Practitioner. She has been working in Rehab in Adelaide (SALHN) since 2002 and specifically with Amputee Patients and their families since 2010. She completed her Masters of Nurse Practitioner in 2014.
Siobhan supports patient who have undergone major Amputations from Southern Adelaide and Country SA on their journey from pre-amputee care through their hospital stay, prosthetic rehabilitation and to ongoing care in the community on completion of their rehabilitation Journey.
